Dear Family Member:

Thank you for considering MEREDITH PLACE to meet the residential needs of your loved one. Please read the following information carefully. Since you are considering admission, you will want to be familiar with a few of the policies and services that pertain to the stay in the facility, and that make our residents, (your loved ones) more comfortable. MEREDITH PLACE believes in a team approach to care in which the staff, residents, and families are all dedicated to the same purpose: To Increase The Independence And Ability Of A Resident To Lead An Active And Fulfilling A Life As Possible.

We attempt to create an atmosphere that is unique. Our resident assistants and direct care staff provide 24 hour supervisory attendance. We contract through other providers and professionals to provide the services needed by our residents. We work toward maximizing the potential of each resident which requires hard work and dedication from everyone on our team. It is easy for our staff to provide assistance to that individual which is difficult for the family to provide, or even difficult of self-care by that individual. We are experienced in handling difficult and easy tasks. Each member of our team - join in an effort to assist the resident in reaching his maximum potential of independence and self-respect.

NOTE: We are not a hospital. We are a RESIDENTIAL CARE FACILITY which assists the activities of daily living for our residents.

MEREDITH PLACE'S motto is "Home is Where the Heart is..." and we aim to become "A Home Away From Home." Our goal is to provide options which will allow individuals to remain independent in our residential care facility.

• Private or shared rooms
• 24 Hour Supervision
• Medication Administration
• Assistance with personal care
• Three (3) balanced meals
• In-between snacks
• Social and recreational activities
• Housekeeping, laundry and maintenance
• Transportation to medical appointments.

All menus are designed by a registered dietician to assure meals are healthy and appetizing.  We offer activities to meet needs and desires of the resident. Social activities will include such things as family gatherings, dinners, making fresh bread, gardening, outings, etc. We provide planned outings for shopping, lunch, theater, picnic, and much more.  After reading the enclosed information, you may have questions about our facility or the services we provide.
